/n Shift :Days; 8a-4:30p; 2-4 shifts per month; open availability;
weekends as-needed /n /n/n /n The community you'll be caring for :
AdventHealth Palm Coast - 60 MEMORIAL MEDICAL PKWY, Palm Coast,
32164 /n /n/n /n The role you'll contribute: /n /n/n /n Evaluates,
develops an appropriate plan of care, and treats patients who
present with bodily impairments and functional limitations as
medically prescribed. /n /n/n /n The value you'll bring to the
team: /n /n/n /n/n /n - Assesses patient ability to safely and
effectively perform functional mobility skills and evaluate muscle,
nerve, joint, and integumentary systems. /n /n/n /n - Designs and
implements a treatment plan of care utilizing practice standards
and recommended techniques and modalities, insurance guidelines,
and patient's needs. Monitors patient's response and progress to
treatment and modifies treatment plan as appropriate. /n /n -
Documents and charges accurately, completely, and timely using
measurable and objective terminology, following standards of
practice, insurance guidelines and department/hospital policies and
procedures. /n /n/n /n - Provides discharge planning assistance and
information regarding appropriate selection/use of adaptive
equipment and follow up services. /n /n/n /n - Communicates
appropriately and effectively with patients, family and healthcare
professionals within and across care settings and communities. /n
/n/n /n/n /n The expertise and experiences you'll need to succeed:
/n /n/n /n Minimum qualifications : /n /n/n /n/n /n - Licensed in
applicable state as a Physical Therapist OR Temporary State permit
as Physical Therapist. /n /n/n /n/n /n Preferred qualifications :
/n /n/n /n/n /n - Needs Acute PT experience (student internship
experience is acceptable) /n /n/n /n/n /n This facility is an equal
