The Pavilion at Crescent Lake - Director of Social Services
Position Summary:
The primary purpose of your job description is to assist in
planning, organizing, implementing, evaluating, and directing the
overall operation of our facility's Social Service Department in
accordance with current federal, state, and local standards,
guidelines and regulations, our established policies and
procedures, and as may be directed by the Director of Social
Services and/or Administrator, to assure that the medically related
emotional and social needs of the resident are met/maintained on an
individual basis.

Essential Functions - Exhibits ability to perform functions of
providing the following;
Assist in planning, developing, organizing, implementing,
evaluating, and directing the social service programs of this
facility.
Assist in the development, administering and coordinating of the
department policies and procedures.
Keep abreast of current federal and state regulations, as well as
professional standards, and make recommendations on changes in
policies and procedures to the Administrator.
Review department policies and procedures, at least annually, and
participate in making recommended changes.
Assist with developing and implementing policies and procedures for
the identification of medically related social and emotional needs
of the resident.
Participate in community planning related to the interests of the
facility and the services and needs of the resident and family.
Participate in discharge planning, development and implementation
of social care plans and resident assessments.
Interview resident/families to obtain a social history.
Perform administrative requirements, such as completing necessary
forms, reports, etc., and submitting such to the Director as
required.
Involve the resident/family in planning social service programs
when possible.
Assist in arranging transportation to other facilities when
necessary.
Refer resident/families to appropriate social service agencies when
the facility does not provide the services or the needs of the
resident.
Provide information to resident/families as to Medicare/Medicaid,
and other financial assistance programs available to the
resident.
Provide consultation to members of our staff, community agencies,
etc., in efforts to solve the needs and problems of the resident
through the development of social service programs.
Coordinate social service activities with other departments as
necessary.
Work with the facility's consultants as necessary and implement
recommended changes as required.
Assist in providing solutions for social and practical
environmental problems including seeking financial assistance,
discharge planning (including collaboration with community
agencies), and referrals to other community agencies when
specialized assistance is required.
Evaluate social and family information and assist in determining
plans for social treatment.
Interpret social, psychological, and emotional needs of the
resident/family to the medical staff, attending physician and other
care team members.
Assist in obtaining resources from the community social, health and
welfare agencies to meet the needs of the resident.
Provide consultation to members of our staff, community agencies,
etc., in efforts to solve the needs and problems of the resident
through the development of social service programs.
Review departmental complaints and grievances from personnel and
make written reports to the Administrator of actions(s) taken.
Folly facility's established procedures.
Assist the QA Committee in developing and implementing appropriate
plans of action to correct identified deficiencies.
Participate in facility surveys made by authorized government
agencies as necessary or as may be directed.
Ensure all charted progress notes are informative and descriptive
of the services provided and of the resident's response to the
service.
Involve the resident/family in planning objectives and goals for
the resident.
Meet with administration, medical and nursing staff, as well as
other related departments in planning social service programs and
activities.
Attend and participate in the Utilization Review Committee
functions as required or as may be necessary.
Attend department head meetings, etc., as scheduled or as may be
called.
Assist in developing a written plan of care (preliminary and
comprehensive) for each resident that identifies the problems/needs
of the resident and the goals to be accomplished for each
problem/need identified.
Encourage the resident/family to participate in the development and
review of his/her plan of care.
Ensure that all social service components of the MDS are completed
and signed in a timely manner.
Assist in the scheduling of care plans and assessments to be
presented and discussed at each committee meeting.
Develop and maintain a good rapport with all services involved with
the care plan to ensure that a team effort is achieved in
developing a comprehensive plan of care.
Be a dedicated member of the Admissions team.
Must be able to participate in weekend/holiday manager
rotations.
Resident Rights/HIPPA Functions:
Maintain the confidentiality of all resident care information
following all standards and guidelines of HIPPA;
Ensure that all residents are treated fairly, and with kindness,
dignity, and respect;
Knock before entering the resident's room;
When speaking on the phone regarding resident information, assure
confidentiality at all times, being careful to be aware of who is
standing near the desk and may overhear conversation;
Report all grievances and complaints made by the resident to the
Administrator.
Other Position Functions:
Perform Administrative requirements such as completing necessary
forms, reports, etc.
Conducts self with professional decorum at all times and with all
persons in the facility.
Follows established Infection Control policies and procedures.
Report all unsafe/hazardous conditions, defective equipment, etc.
to Administrator immediately.
Ability to speak, read and write the English language in a legible
and understandable manner.
Shall not pose a direct threat to the health or safety of other
individuals in the workplace.
Possesses the ability to make independent decisions when
circumstances warrant such action.
Must possess the ability to deal tactfully with personnel,
residents, family members, visitors, government agencies/personnel
and the general public.
Maintains compliance with all HIPAA regulations.
Ability to accept assigned duties in a cooperative manner.
Ability to assume accountability for all data contained in the
employee's handbook.

Must demonstrate the ability to relate positively, effectively, and
appropriately with residents, families, community members,
volunteers, and other facility staff.
A special interest in working with long-term residents and the
elderly is preferred.
Working knowledge of office machines, such as copiers, scanners,
faxes, phone systems, etc.
General computer literacy and good knowledge of Microsoft Outlook,
Excel, Word, and Google
Must have excellent attention to detail.
Must attend and participate in continuing education programs and
all mandatory in-services.
Must be in good general health and demonstrate emotional
stability.
Education: High School Diploma or equivalent. Bachelor's Degree
from an approved school of Social Work is preferred.&a
