Job Description:
H2 Health is seeking a qualified and compassionate Physical
Therapist to join our dynamic team. As a leader in rehabilitation
services, we provide a comprehensive range of therapies, including
physical, occupational, and speech, tailored to meet the needs of
our diverse clientele. Our Physical Therapists are critical in
implementing personalized care plans, aimed at improving patient
outcomes, ensuring functionality, and enhancing quality of life. We
offer therapy sessions both at home and in well-equipped
facilities, adapting to patient needs to maximize recovery and
independence. Assess patient condition by conducting physical tests
and evaluations to determine muscle strength, joint flexibility,
and motor function. Develop treatment plans by identifying patient
requirements and goals, tailoring interventions and therapy
activities accordingly. Implement physical therapy interventions,
including therapeutic exercises and manual therapy techniques, to
assist patient recovery and enhance mobility. Document patient care
and progress by maintaining accurate and timely case notes and
clinical reports. Educate patients and their families about
treatment plans, therapeutic exercises, and strategies to improve
well-being at home. Collaborate with other healthcare professionals
to ensure comprehensive care delivery. Stay updated with
advancements in the field and incorporate new techniques and
equipment as necessary. Expertise in developing personalized
rehabilitation programs Strong interpersonal and communication
skills Proficient in using physical therapy tools and equipment
Ability to make critical decisions regarding patient care Emphasis
on delivering patient-centered care Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current state
licensure as a Physical Therapist. BLS/CPR certification.
