Job Description:

As a Catastrophic Nurse Case Manager at {EMPLOYER-DESC}, you will be tasked with managing complex patient cases resulting from catastrophic events, such as severe injuries or illnesses. This vital role ensures that patients receive optimal and cost-effective healthcare and support throughout their recovery process. You will work closely with healthcare providers, patients, and insurance companies, coordinating comprehensive care plans that address the unique needs of each patient. Evaluate new cases to determine severity and required medical interventions Develop and implement comprehensive care plans in collaboration with multidisciplinary teams Monitor patient progress, updating care plans and treatment strategies as necessary Liaise with healthcare providers, insurance companies, and other involved parties to ensure smooth communication and service delivery Educate patients and families about the care plan and associated medical information Maintain detailed and accurate case documentation for legal, medical, and insurance purposes Advocate for patients’ needs to ensure access to necessary healthcare services Conduct home or hospital visits as required to assess patient condition and needs Strong clinical knowledge in nursing and patient care management Excellent organizational and project management skills Profound ability to assess, plan, and coordinate complex health care plans Superb communication and interpersonal skills Critical thinking and problem-solving abilities Knowledge of healthcare regulations and insurance processes Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid nursing license Certification in Case Management (e.g., Commission for Case Manager Certification [CCMC]) preferred
