Job Description:
Encompass Health is seeking a dedicated Pharmacist to join our
team of healthcare professionals in delivering top-tier medical
care with empathy and expertise. Our network of inpatient
rehabilitation hospitals serves as a critical step for patients
aiming to regain independence post serious illness or injury. The
ideal candidate will be responsible for managing medication therapy
and overseeing pharmacy operations to ensure the safety and health
of our patients. With a focus on patient outcomes, the Pharmacist
will work alongside a collaborative medical team to optimize
treatment plans and promote recovery. Assess the status of
patients’ health conditions to ensure appropriate medication
management Collaborate with healthcare team members to develop
effective patient treatment plans Supervise the preparation and
dispensation of medications Monitor drug therapy regimens for
contraindications, drug-drug interactions, drug-food interactions,
allergies, and appropriateness of drug and dose Provide information
and advice regarding drug interactions, side effects, dosage, and
proper medication storage Adhere to all relevant regulations
regarding drug dispensing and security Maintain accurate records
for pharmacy controlled substances and ensure compliance with
regulatory requirements Participate in clinical programs and
initiatives aimed at improving patient outcomes and efficacy of
therapy Strong knowledge of pharmacology and pharmaceuticals
Skilled in pharmaceutical therapy management Ability to work
effectively in a team Strong organizational skills and attention to
detail Excellent communication and counseling skills Medical
Insurance Dental Insurance Vision Insurance 401k Active Pharmacist
licensure in the state of practice Board certification in
Pharmacotherapy (BCPS) or similar, preferred
